Identify other residential settings for effects of isolating

Purpose
You are completing this survey to help the Minnesota Department of Human Services (DHS) identify providers who may need to show how they meet the requirements of the new home and community-based services (HCBS) rule. This process is known as "provider attestation."

Background
The federal government requires states to identify any residential settings that may have the effects of isolating people with disabilities from the community. This includes residential settings that group or cluster people with disabilities who receive waiver-funded home and community-based services. The state must then assess whether the setting complies with the requirements of the HCBS settings rule. Earlier this year, DHS assessed provider-controlled, licensed residential settings, including:

o Adult foster care or supportive living services provided in a licensed setting
o Child foster care or child supportive living services provided in a licensed setting
o Customized living
With today's survey, DHS is focusing on other residential settings that may have the effect of isolating people with disabilities, such as single providers who offer supported living services or independent living skills to several people with disabilities who live in the same apartment building. Once identified, these providers will follow the same attestation process to determine whether their services isolate people with disabilities from the community.
